1. The result that a former 5 years Plan arrived at.

Medical coop commitee is to decide a new 5 years plan in a general assembly of June 1995. The plan started from 1988 for the first 5 years, and it came to an end in 1992, and this plan let whole country 4 objects advance greatly, and this plan pursued the formation of medical care consumers cooperative which seemed to be medical care consumers cooperative thoroughly and brought various kinds of progress to the medical care consumers cooperative movement whole. We increased a member of coops more than 100, 000 households each year , and it gave the prospects cutting situation around medical care subjectively that an activity to increase the number of member and participation in various kinds of activities. The crucial issue for medical care consumers cooperative,s development in 21 st century is "cooperation and self-government based on participation of members". The development of this movement really too bring the new members and person whom medical care engaged in , and they carry a democratic change in Japanese medical care.

2 An aim and a point of new 5 years plan

New 5 years plan aims to reinforce the ability opening up the situation subjectively on leap for a purpose , and this concrete contents for the purpose is to display the advantage of cooperative social system of medical care consumers cooperative built till now both in movement and the spot of medical service production. Firstly the new plan makes clear contemporary social role of medical care consumers cooperative in the next three field.

1 Step up of so-called medical care administration reform line and the role of medical care consumers cooperative. And it becomes little that region inhabitant assists mutually. People seeks new cooperative relationship. In the medical care institutes of medical coop, members and persons whom medical care engage in cooperate to play this function. It is neccesary to accumulate knowledge and skill about health, to increase members and develope various medical care activities in order to make a region more healthy.

"Organization activity"

Medical Co-op Committe of JCCU has 1,8 million households members. Many medical care consumers cooperatives organize household of 20 - 30 % of their chatchment areas, and some go over 50 % . In eight prefectures, whole medical care consumers cooperatives in its prefecture organize more than 10 % of total household.
With regard to such arrival point , new 5 years plan aims that all medical coop organize over 50 % households in their catchment areas, branches should be set up each unit area , by 2000 all medical care consumers cooperative organize 3 million household whole countru. It is required to achieve this goal that we reinforce an activity at a han-group and a branch, continue an activity as before, and develope various works which give charm to broad resident and correspond to needs of them.

We must increase members at a tempo of 2 times of result as before to achieve a goal. We regard the 4 national objects as a most important and tackles it . (The 4 national objects are increasing members , increasing han-groups , educating active members and increasing investment) . It becomes more and more important to cheek eating habits and whole life style through 6. 9. 2. movement (han-group meeting to be held in June , September and February) and han-groups in case of a fall of protect power of region and family. We aim to increase frequencyof han-group meeting , to get over 50% member participate a han-group meeting. Increasing active members who have conviction at a contemporary activity and arrival point of medical care consumers cooperative to several times too is required to achieve these objectives. We examine and enforce school system like health university, social and consumers cooperative school which have been giving a place of learning of members. And, we establish correspondence education for members too and magnify an opportunity to learn. "Health medical care activity"

We aim to get rich and full results in the field of health-medical care activity being handed down from varios activities till now. There are 4 important points to get results.
First , we make self sovereignty of health a basis , and aim at principal objective to what we protect a right living in health of members , and develop practice of "charter of patients, right".
Second , we reinforce activities to form healthy life and community. We rise aiming at development of activities put mainly on health medical care activity worthy of medical care consumers cooperative.
Third , we reinforce outpatient medical care on leap and raise ability to cope with energency case in shape suitablefor each hospital and clinic. At the same time, our organization do best to prevent and control chronic diseases holding central position in the future in daily life. We extensively make use of health checkups that are property of medical care consumers cooperative, and "han-group based health university patient society, meeting" as place of learning. Patient society will educate a lot of "honor student of self control" who are to be expert of illness of oneself. At the same time , patient society is usefull for pleasant treatment life.

In order to operate buisnesses corresponding new demand of members , it is indespensable to secure a stable income and surplus balancing with reproduction.
Under the circumstance of steady retreat of social security system, demand for medical care means a conprehensive provision of health, medical and welfare services. We make much of primary medical care, especialy we strengthen outpatient care , at the same time we improve inpatient care including institution improvement. We strengthen a movement leting local authority, health checkups, and raise specific gravity as the cooperative utilization buisness that is most easy to participate for members. We develope many buisness like the 4 programmes based on the "Health and Medical Service for the Aged law". We also tackle a formation of network of safety for the aged, develope institutional care.
From now on, we examine new buisness in which volunteers participate and rise aiming at management activity respondihg new demand for medical services. We make clear the importance of management improvement , we strengthen healthy management activity and financial constitution through budgetting in each section, prime cost management, 5% surplus , realization of investment ratio 20%. We are going to aim at the management activity that "institution expansion institution preparation receiving a period in advance" can realize from "institution plan pressed for embodiment grieved at stopping with deterioration".

"Education activity"

We rise aiming at establishment of systematic education system of members with a prospect for 21 century. In the various kinds of activities that it is "learned and educate each other" , we guarantee learning of members systematicaly. In addition to a place of education as before, we spread the "correspondence education" that aimed at a activ member on a large scale. In both place of activity and the medical care spot, display of members, right to learning is a key word of development of medical care consumers cooperative advance.
On the other hand , we develope staff education too. In this case, based on independent learning we push a human development through work and medical care consumers cooperative movement.

Through the estabilishment of "work place with vitslity" based on the pursue of the "charter of patients, right", we tackle to organize a relationship in which staff and member can grow up jointly , and feed individual ability to achieve organization aim constructing of relation that can be met and raise an ability as a group.

"International activity"

We participate in the World Summit for Social Development and NGO Forum held in Copenhagen in March, 1995. We also attend to the second international health medical cooperative forum in September. We plan for systematic reinforcement of international activity during we accumulate various forms of exchanges. We plan for reinforcement of Medical Co-op Committee and administrative council and special committee. And, we magnify suporting function that offer useful know-how to develope joint purchase buisness of each medical coop and Medical Co-op Committee.
